สาเหตุของโรคจอประสาทตาอักเสบเกิดจากสาเหตุทั้งการติดเชื้อและไม่ติดเชื้อ ดังนี้ จอประสาทตาอักเสบที่มาจากการติดเชื้อ โรคจอประสาทตาอักเสบเกิดจากตั้งแต่เกิดหรือเกิดขึ้นทีหลัง โดยมาจากเชื้อ ดังนี้ การติดเชื้อแบคทีเรียเช่น วัณโรค ซิฟิลิส การติดเชื้อราโดยเฉพาะที่เกิดจากเชื้อราแคนดิดา การติดเชื้อไวรัสเช่น เริม เอชไอวี โรคหัดเยอรมัน การติดเชื้อปรสิตเช่น โทโคพลาสโมซิส ซึ่งอาจมาจากแมว โรคภูมิต้านทานผิดปกติเช่น โรคเบห์เช็ท และซาร์คอยโดซิส จอประสาทตาอักเสบที่ไม่ได้มาจากการติดเชื้อ โรคจอประสาทตาอักเสบ ที่ไม่ได้เกิดจากการติดเชื้อ ได้แก่ อาการบาดเจ็บบริเวณดวงตา โรคภูมิต้านทานผิดปกติและโรคอักเสบเช่น โรครูมาตอยด์ มะเร็งบางชนิดเช่น มะเร็งต่อมน้ำเหลือง     การวินิจฉัยโรคจอประสาทตาอักเสบ ในการวินิจฉัยโรคจอประสาทตาอักเสบก่อนทำการรักษา จักษุแพทย์จะตรวจตาอย่างละเอียด โดยมีแนวทางการวินิจฉัย ดังนี้ การขยายม่านตาและตรวจจอประสาทตาด้วยกล้องจุลทรรศน์ (Slitlamp)โดยจักษุเเพทย์เพื่อดูจอประสาทตาและเส้นเลือดที่เลี้ยงจอประสาทตา รวมถึงการตรวจวัดความดันลูกตา การตรวจจอประสาทตาด้วยเทคนิค Optical Coherence Tomography (OCT)เป็นการวัดความหนาของจอประสาทตาและการบวมของจอประสาทตาหรือ ดูขั้วประสาทตาได้อย่างชัดเจน การตรวจจอประสาทตาด้วยการฉีดสารเรืองแสงฟลูออเรสซีนเพื่อดูการไหลเวียนของเลือดและการรั่วของหลอดเลือด การตรวจจอประสาทตาด้วยการฉีดสารเรืองอินโดไซยานีนกรีน(ICG Angiography)เพื่อดูการทำงานของหลอดเลือดขนาดเล็กในจอประสาทตา     จอประสาทตาอักเสบ รักษาได้อย่างไร การรักษาจอประสาทตาอักเสบ ทำได้ทั้งแบบใช้ยาและผ่าตัด โดยแพทย์จะพิจารณาตามระดับความรุนแรงของอาการ โดยมีรายละเอียดดังนี้ การใช้ยา การรักษาจอประสาทตาอักเสบด้วยการใช้ยา โดยปกติแล้วจะใช้ยาตามสาเหตุที่ทำให้เกิดอาการ โดยมีตัวยาที่จักษุแพทย์พิจารณาจ่ายให้ผู้ป่วยโรคจอประสาทตาอักเสบ มีดังนี้ การให้สเตียรอยด์ทางหลอดเลือดดำเป็นการรักษาเริ่มต้นมาตรฐานสำหรับผู้ป่วยส่วนใหญ่ที่เป็นโรคจอประสาทตาอักเสบและเส้นประสาทตาอักเสบ ช่วยลดการอักเสบของเส้นประสาทตาและช่วยเร่งการฟื้นตัวของการมองเห็น สเตียรอยด์ชนิดรับประทานในบางกรณีอาจมีการสั่งจ่ายสเตียรอยด์ชนิดรับประทานหลังจากการรักษาทางหลอดเลือดดำครั้งแรก เพื่อลดการอักเสบและป้องกันการกลับมาเป็นซ้ำ การแลกเปลี่ยนพลาสมา (PLEX)หากสเตียรอยด์ทางหลอดเลือดดำไม่ได้ผล อาจพิจารณาทำ PLEX โดยเอาส่วนที่เป็นของเหลวในเลือด (พลาสมา) ออก จากนั้นแทนที่ด้วยสารทดแทนพลาสมา หรือพลาสมาที่ได้รับบริจาค ยาปรับเปลี่ยนโรค (DMTs)หากโรคเส้นประสาทตาอักเสบเกี่ยวข้องกับโรคประจำตัว เช่น โรคปลอกประสาทเสื่อมแข็ง อาจแนะนำให้ใช้ DMTs เพื่อจัดการกับโรคประจำตัวและป้องกันไม่ให้เกิดโรคจอประสาทตาอักเสบในอนาคต   การผ่าตัด การรักษาจอประสาทตาอักเสบด้วยการผ่าตัด จักษุแพทย์จะพิจารณาผ่าตัดวุ้นตา (Vitrectomy) ในกรณีที่ผู้ป่วยมีอาการรุนแรงและไม่สามารถควบคุมอาการได้ด้วยวิธีการจ่ายยาทั่วไป รวมถึงผู้ป่วยที่มีอาการแทรกซ้อนอื่นๆ อย่าง มีเลือดออกในวุ้นตา หรือมีพังผืดดึงรั้งจอประสาทตา เป็นต้น โดยการผ่าตัดวุ้นตา (Vitrectomy) คือการผ่าตัดภายในลูกตา เพื่อนำวุ้นตา (Vitreous) ที่เป็นของเหลวใสภายในลูกตาออก และกำจัดสิ่งแปลกปลอมหรือเนื้อเยื่อที่ผิดปกติที่อาจกดทับหรือดึงรั้งจอประสาทตาออกไป โดยหลังจากการผ่าตัด การอักเสบของจอประสาทตาจะลดลง ลดอาการระคายเคืองของจอประสาทตา หรือในบางกรณียังช่วยฟื้นฟูการมองเห็นที่สูญเสียจากจอประสาทตาอักเสบได้ด้วยเช่นกัน     จอประสาทตาอักเสบสามารถป้องกันได้ไหม?
